Blood-pressure monitoring as an actionable first step
Even where a family decides not to pursue a formal diagnosis, “not knowing” does not have to mean “doing nothing.”
Blood-pressure monitoring emerged as the clearest immediate and actionable measure.
Hypertension can occur in children with ADPKD, and it is treatable. Participants broadly supported regular blood-pressure checks for children known to be at risk, regardless of whether they have undergone genetic testing or imaging.
However, the practical route will differ between countries. In some healthcare systems, children see a paediatrician regularly and blood pressure can be incorporated into routine care. Elsewhere, there may be no established mechanism for monitoring an asymptomatic child. Participants proposed gathering information about current national practice and developing practical recommendations that work across different healthcare settings.
Importantly, families should understand in advance what will happen if high blood pressure is found. Confirmation of paediatric hypertension normally requires further assessment and may lead to imaging or other diagnostic investigations. Monitoring must therefore be accompanied by appropriate counselling and access to expert advice.
